ABSTRACT

Effective methods for visualizing neurovascular morphology are essential for understanding the normal spinal cord and the morphological alterations associated with diseases. However, ideal techniques for simultaneously imaging neurovascular structure in a broad region of a specimen are still lacking. In this study, we combined Golgi staining with angiography and synchrotron radiation micro-computed tomography (SRμCT) to visualize the 3D neurovascular network in the mouse spinal cord. Using our method, the 3D neurons, nerve fibers, and vasculature in a broad region could be visualized in the same image at cellular resolution without destructive sectioning. Besides, we found that the 3D morphology of neurons, nerve fiber tracts, and vasculature visualized by SRμCT were highly consistent with that visualized using the histological method. Moreover, the 3D neurovascular structure could be quantitatively evaluated by the combined methodology. The method shown here will be useful in fundamental neuroscience studies.

ABSTRACT

Tripartite motif (TRIM) family proteins are important effectors of innate immunity against viral infections. Here we identified TRIM35 as a regulator of TRAF3 activation. Deficiency in or inhibition of TRIM35 suppressed the production of type I interferon (IFN) in response to viral infection. Trim35-deficient mice were more susceptible to influenza A virus (IAV) infection than were wild-type mice. TRIM35 promoted the RIG-I-mediated signaling by catalyzing Lys63-linked polyubiquitination of TRAF3 and the subsequent formation of a signaling complex with VISA and TBK1. IAV PB2 polymerase countered the innate antiviral immune response by impeding the Lys63-linked polyubiquitination and activation of TRAF3. TRIM35 mediated Lys48-linked polyubiquitination and proteasomal degradation of IAV PB2, thereby antagonizing its suppression of TRAF3 activation. Our in vitro and in vivo findings thus reveal novel roles of TRIM35, through catalyzing Lys63- or Lys48-linked polyubiquitination, in RIG-I antiviral immunity and mechanism of defense against IAV infection.

OBJECTIVE@#Aiming at the different characteristics of the various stages of medical equipment life cycle in hospital, research on the targeted and meticulous management mode.@*METHODS@#Divides the whole life cycle of medical equipment in hospital into four phases, which are the selection demonstration period, purchase acceptance period, maintenance period, and retirement disposal period, and comparison with human fetal period, infant stage, adult stage and old age.@*RESULTS@#With the meticulous management mode, the service quality of medical equipment in hospital has been improved, and the service benefits have been enhanced.@*CONCLUSIONS@#According to the respective characteristics of different stages, the corresponding meticulous management mode is implemented to make the management more scientific and standardized, and the operation is safer and more reliable, which escorts the whole life cycle of medical equipment in hospital.

ABSTRACT

Ewing's sarcoma is a kind of bone and soft tissue tumor which is highly invasive and mainly occurres in children and adolescents.In recent years,combined chemotherapy,surgery and radiation therapy in treatment of Ewing's sarcoma,patients' prognosis and life quality have been significantly improved.However,over the past 20 years,the treatment of Ewing's sarcoma entered the platform period.The 5-year overall survival rate remained at 55%-75%.Multiple metastasis and recurrence are the main factors of poor prognosis and death.Chemotherapy,radiotherapy and molecular targeted therapy are still the main methods for the treatment of Ewing sarcoma.The side effects,drug resistance and the use of the combination regimen of antitumor drugs have been plaguing the clinical workers.In order to improve the efficacy of chemotherapy drugs and reduce the toxic side effects,Multi-disciplinary and multi-center clinical studies on Ewing's sarcoma patients who suffered from local control or recurrence have been launched by Domestic and European and American countries.As an important supplementary mean for the treatment of Ewing's sarcoma,patients often appear a series of complications after radiotherapy,including the risk of local damage or secondary tumors.Therefore,it is necessary to further clarify the indications of radiotherapy and the timing of preoperative and postoperative radiotherapy.The specific chromosome translocation and the expression of the fusion gene EWS/FLI 1 have been found in Ewing sarcoma.Nevertheless,the mechanisms that drive tumor relapse and metastasis remain unknown.Molecular targeted therapy can be used to inhibit tumorigenesis and progression by regulating the upstream or downstream target genes of EWS/ FLI1.In conclusion understanding of the current treatment status of Ewing's sarcoma,results of multi-center clinical trials and theory of genomics research will contribute to the design of new biological therapies so as to establish individualized treatment modalities.In this paper,we present a review on the progress of Ewing sarcoma chemotherapy,radiotherapy,molecular targeted therapy and immunotherapy.

ABSTRACT

Objective To investigate reasonable method of deciding internal target volume (ITV) by comparing physical phantom volumes (including moving volume) with reconstruction volumes of spiral CT scanning.Methods The various-volume wax blocks which were labeled No.1-9 were made and put on the respiratory motion simulator.The range of motion was set 2.5 cm and frequency 18 beats/min.All blocks were scanned 10 times continuously and imported into the Eclipse TPS.All blocks volumes were calculated and then compared with the true physical volumes and paired t-test.Results The reconstruction volumes of 1-9 blocks were bigger than their stationary volumes (121.77 cm3 vs.103.14 cm3,P =0.038),but significantly smaller than their moving volumes (121.77 cm3 vs.161.75 cm3,P =0.045).The results can be gotten in different volume block scanning.The relative deviation of reconstruction volumes and the moving volumes tends to increase as the stationary physical volume decreases.Conclusions As to moving targets,conventional spiral CT scanning speed is too fast to collect all volume information of targets.So the one-time-scanning volume does not represent the ITV.

ABSTRACT

Objective The incidence rate of sacrococcygeal tumor is low , however , due to its special position , clinical symptoms are inclined to be more severe .The aim of the article was to explore the methods and therapeutic effects of operative treat-ment for sacrococcygeal tumors . Methods A retrospective analysis was made on 44 cases of sacrococcygeal tumors treated in our hospital from January 2008 to December2012 .Of all the cases , there were 11 chordomas , 9 neurinoma , 4 malignant fibrous histioto-ma, 4 giant cell tumor, 7 metastases tumors and 9 others.All the patients had definite pathological diagnosis after operation .Operative treatments involved simple resection of rumors , simple resection of lesions and resection of lesions plus screw-rod system internal fixa-tion. Res ults No patients died in perioperative period .Follow-up was made in all patients for the average time of 3.3 years(4 months to 59 months).Except for 4 patients'delayed healing of incision operation and 1 patient′s healing of operation incision after de-bridement , all the other patients healed after enhancing the wound dressing , among which there are 3 cases of chordoma recurrence , 1 case of giant cell tumor recurrence , 3 death cases of malignant fibrous histiotoma in 2 years and 1 case alive with tumor .All patients with metastases tumor died in 3 years.Except 1 patients with osteosarcoma alive with tumor , all patients′symptoms had been alleviated after operation. Conclusion Chordoma and giant cell tumor still have a high recurrence rate after operation . The extent of rumor resection and nerve preservation are determined by the range of tumors.Preoperative embolization of the internal iliac artery can clearly reduce bleeding and improve the operation safety .

ABSTRACT

Objective To study the effect of P style of total gastrectomy and digestive canal reconstruction on quality of life and C-reactive protein (CRP) in serum.Methods 90 patients with total gastrectomy were randomly divided into the control group (n =45 cases) and the observation group (n =45 cases).The patients in the control group were operated through Lahey + Braun,while the patients in the observation group were operated through P style of total gastrectomy and digestive canal reconstruction.The quality of life and serum CRP level were evaluated after treatment for 6 months.Results After operation,the quality of life was increased in two groups,and the quality of life in the observation group was higher than that in the control group (t =3.4201,P < 0.05).After operation,the CRP level was decreased in two groups,and CRP level in the observation group was higher than that in the control group (t =6.3442,P < 0.05).Conclnsion P style of total gastrectomy and digestive canal reconstruction can improve quality of life and decrease CRP.

ABSTRACT

Objective To evaluate effect and safety of first-line chemotherapy regimen combined with rh-endostatin for osteosarcoma of the extremities.Methods Sixty three patients with osteosarcoma were randomly divided into experiment group and control group.There were 32 patients in experiment group,and 31 patients in control group.Nine patients 9 were rejected because they did not meet the standard.Finally,54 patients were enrolled in this study,including 29 patients in experiment group,and 25 patients in control group.In the experiment group,the patients were treated with rh-endostatin combined with MTX,IFO,DDP,and ADM,while patients in control group were treated with MTX,IFO,DDP,and ADM.Several indexes including median progression-free survival time,clinical benefit rate,progression-free survival rate,limb salvage rate,and survival rate were used to assess clinical effect.The safety of rh-endostatin was evaluated by comparing incidence of adverse events in the two groups.Results The median progression-free survival time of experiment group and control group was 18.9 months and 13.1 months,respectively; there was no significant difference.In the experiment group,the clinical benefit rate,progression-free survival rate,survival rate and limb salvage rate were 89.7%,37.9%,65.5%,and 89.7%,respectively; while in the control group,the clinical benefit rate,progression-free survival rate,survival rate,and limb salvage rate were 88.0%,36.0%,68.0%,and 96.0%,respectively.There was no significant difference in 4 indexes mentioned above between two groups.Conclusion After being combined with first-line chemotherapy regimen,rh-endostatin doesn't show significant advantage in controlling tumour progression and improving survival rate.No more toxicity and new side effects are found after using rh-endostatin.

ABSTRACT

Objective:To compare the dosimetric parameters between three-dimensional conformal and intensity-modulated radiotherapy for cervicothoracic spine metastases.Methods:Totally 10 cases of patients with cervicothoracic spine metastases were devided into 2 groups:3 patients with vertebral metastases;the other 7 patients with vertebral+annex metastases.The prescription dose for gross target was 60 Gy,D_(max) of spinal cord≤50 Gy,D_1≤45 Gy.Each patient was designed.One set of 3DCRT plan and three sets of IMRT plan were designed for each patient.We used 6 beams for 3DCRT plan,including 2 beams for the upper neck.3 sets of Step and shoot IMRT plans were consisted of 5-fields,7-fields and 9-fields,respectively,with equal angle and coplanar.DVH.CI and HI dose distributions of PTV and OARs were compared between 3DCRT and different set of IMRT plans.Results:(1)When the gross target included only vertebral,the CI of 3DCRT plan was lower than that of IMRT plan,but the HI of3DCRT plan was higher than that of IMRT plan.Moreover,5-fields IMRT plan was better than other plans.(2)When the gross target included vertebral and annex,the D_(80)、D_(mean) of IMRT PTV were higher than that of 3DCRT.As for spinal cord,5-fields D_(max)<50Gy,7-fields D_(max)=49 Gy-51 Gy,9-fields D_(max)>50 Cry.The CI of 3 sets of IMRT plans were 0.69,0.66,0.69,respectively.The CI of 3DCRT PTV was 0.53,worse than that of IMRT(P<0.05).The average segments of 3 sets of IMRT plans were 83,104,160,and the average Mu of IMRT plans were 727,755,953,respectively.Conclusion:Compared with 3DCRT plan,the dose distribution,OAR and CI Of MRT plans were better.5-fields IMRT plan is recommended strongly.

OBJECTIVE@#To explore the relationships between the sphenoid-sinus-o-tumoropathy and local anatomy, as well as the characteristics of invasive damages of nerve system in sphenoid-sinus-o-tumoropathy, and emphasize that it is crucial of early diagnosis and surgery for sphenoid-sinus-o-tumoropathy.@*METHOD@#Retrospective analysis of the characteristics of invasive damages of nerve system in 13 patients with sphenoid-sinus-o-tumoropathy, and follow-up the outcomes of nerve system after transnasal endoscopic surgery.@*RESULT@#1) The majority clinical manifestations of invasive damages in nerve system are cranial nerves, there are 9 cases optic nerve, 7 cases oculomotorius nerve, 4 cases trochlear nerve, 4 cases abducent nerve and 1 cases trigeminal nerve have been found damage at different levels in this group, and the secondly are 3 cases dysfunction of pituitary and 2 cases central nerve damage. 2) The improvement of invasive damages in nerve system after transnasal endoscopic surgery are as follow: at the day of operation or the next day, there are 4 cases to be reported that the symptoms of headache and eyes relieve very well; during 3 to 7 days after operation there are 9 cases to be reported that the symptoms of headache and eyes relieve or disappear significantly; 2 weeks after operation, there are 8 cases to be reported that the optical sights recovery at different levels, and there are many symptoms such as headache (9 cases), visual diplopia (1 cases), ptosis (2 cases), fixation of eyeball (1 cases), exophthalmoptosis (2 cases), face pain (2 cases), orbital paralysis (2 cases) disappeared completely. No complications of cerebrospinal fluid rhinorrhea, brain edema and hemorrhage to be found during the period of operation and follow-up.@*CONCLUSION@#The majority clinical characteristics of invasive damage in nerve system with sphenoid-sinus-o-tumoropathy are cranial nerve manifestations, and the second manifestation is dysfunction of pituitary. Transnasal endoscopic surgery could inhibit the invasive damages in cranial nerves of sphenoid-sinus-o-tumoropathic patients effectively.

ABSTRACT

Objective: To explore the position change of the setup reference markers on the sternum due to the change of hands grip position. Methods: Six hands grip positions were simulated for 8 patients and the set-up reference points marked on the sternum skin were compared while the hands grasped different positions of the hands. Results: The setup markers on the sternum were deviated by all five hands grip positions. The maximum deviated distance was 14mm. Conclusion: The change of the hands grip position can affect the position of setup reference markers on the sternum of the patients and thus the isocenter of plan may be aligned to the wrong area.

ABSTRACT

Objective To study the mechanism of Helicobacter Pylori inhibiting the healing of acetic acid - induced gastric ulcer in rats. Methods Rats were infected with Helicobacter Pylori and the model of acetic acid gastric ulcer was replicated at 4 weeks after in-fection. Amount of G cell and D cell in mucosa of gastric antrum, quantity of gastric juice and pH were measured at the 3rd,Sth, 16th day after the model was replicated. Results When the group of Hp + acetic acid ulcer compared with the group of acetic acid ulcer, the number of G cell, quantity of gastric juice increased (P < 0.01), and the number of D cell and pH decreased (P < 0.01). Conclusion Helicobacter Pylori inhibits ulcer healing through increasing gastric acid secretion.

In order to know that Streptococcus exist in health swine herd,2 204 cases of nose swabs were collected in swine from different areas in Dongbei three province of northeast China.Seed swab to Streptococcus culture medium,24 hours after culturing,gram staining with becteria culture,gram positive Streptococcus were identified with PCR technology,then farther identified serotype with Streptococcus suis 1,2,7,9 serotype special primers.The results showed that Streptococcus exist rate in swine herd of Heilongjiang,Jinlin,Liaoning is 29 %,27 %,34 %,respectively.PCR typing assay indicated that 155 strains of Streptococcus suis were isolated including 7 strains of 1 serotype,39 strains of 2 serotype,4 strains of 7 serotype,11 strains of 9 serotype,94 strains of other serotype in three province of northeast China.

ABSTRACT

Objective To measure the distortion of digital images generated by radiotherapy simula-tor,and to study the appropriate method of correction. Methods The grid correction plate and Microsoft Visual C + + 6.0 were used for correction. The area error and boundary maximum displacement error of dig-ital images before and after correction were calculated. The post-correction images were compared with film images to evaluate the correction method. Results The area error was 0.31% - 12.36%, and the bounda-ry displacement error was more than 0 -6 mm for 4 cm ×4 cm - 12 cm × 12 cm radiation field before correc-tion. For commonly used radiation field(12 cm × 12 cm) ,the post-correction area error and the boundary displacement error were 0.48% and 0.46 mm,respectively. Conclusions The least square and polynomi-al fitting correction method can fulfill the requirement of conventional radiotherapy.

ABSTRACT

Objective To study the effect of various methods determining lung volume and fraction dose on the lung dose-volume parameters for lung cancer patients. Methods Twenty patients with lung cancer were rantdomly enrolled into our study and the plan of three dimensional eonformal radiation therapy(3DCRT)was designed by Varian Eclipse TPS.The lung volumes and the dose-volume parameters were measured under CT value of-300- -980, -400- -980 and -500- -980.Under CT value of -400- -980,total lung volumes were confirmed.The dose-volume parameters of V30,V20,V10 and mean lung dose(MLD)were reevaluated after GTV,CTV and PTV were subtracted from the total lung volumes and when the fraction dose was elevated from 2.0 Gy to 10.0 Gy. Results When the CT value ranged from-300--980 to-500--980,the median reduction of the total lung volumes(-9.10%)was significantly higher than that of V30,V20,V10 and MLD(-3.18%,-1.13%,0.82%and-0.79%,respectively).When the total lung volume was fixed at CT value of-400--980,the alterations of V30,V20,V10 and MLD became more apparent as the increase of the subtracted lung volume,among which the alteration of V30 was most significant while V10 the least.Among five cases with a fixed total dose of 60 Gy and PTV less than 140 cm3,the V30,V20,V10 and MLD were increased to a similar extent(about 40%)when the fraction dose was increased from 2.0 Gy to 10.0 Gy.MLD was increased(36%)when the fraction dose was above 6.0 Gy. Conclusions When CT value ranges from-300- -980 to-500- -980,the total lung volume is influenced most.The alteration of V30,being statistically significant,might have some significance but is not enough to determine the plan of radiotherapy clinically.The alteration of V20、V10 and MLD is not statistically significant.When the overlapped target volume is subtracted from the total lung volumes,the alteration of V30 is the most sign:tifhcant while V10 the least.The fraction dose,being the most consuming factor(>10%)when comparing with the CT valHe and targeted volume,can significantly influence the dose-volume parameter.

ABSTRACT

OBJECTIVE To discuss the preoperative locating diagnosis and transnasal endoscopic surgery for cerebrospinal fluid rhinorrhea in sella turcica region.METHODS All of the 22 patients with cerebrospinal fluid rhinorrhea had been located the position of leakage by means of high resolution computed tomography(HRCT),magnetic resonance imaging(MRI)and/or computed tomography cisternography(CTC)techniques,and then the skull base defects were repaired with fascia via transnasal endoscopic surgery.All the patients were followed up for 6 to 51 months.RESULTS Nineteen cases were successfully repaired by operation once and another patient was cured through reoperation.Two cases refused reoperation after failure of the operation and lost follow-up.No complications such as intra-cranial infection and hemorrhage were found during the perioperative period and follow-up.The main failure causes of operation were the necrosis of the repairing fascia,missing the real leakage,and the recurrence of malignant tumor.CONCLUSION It is a key step to locate leakage accurately before operation by means of CT and MRI.Transnasal endoscopic technique is the optimal choice for management of cerebrospinal fluid rhinorrhea in sella turcica region.
